Hospital Discharge Planning
Our Occupational Therapists communicate in a timely manner with medical teams to determine the necessary supports for clients, allowing them to make a smooth transition back to community life and engagement. We recommend the most effective adaptive equipment and care supports to keep our clients safe at home while recovering from their injuries.
Wheelchair/Power Mobility
Creative Therapy’s Occupational Therapists are committed to ongoing training regarding the most current power mobility options and technology available to our clients. We take a holistic approach to power mobility assessments including the client’s function, goals, and environment.
Case Management Services
Our skilled Occupational Therapy case managers collaborate with individuals and funders to coordinate and implement the most appropriate, timely, and cost-effective rehabilitation services and supports. Our Occupational Therapists help clients navigate the information provided by various members of the rehabilitation team, in addition to ensuring funders and stakeholders stay well-informed.
